APPLICATION TO BEGIN SERVICE
1. Contact our office to start the process

Our business hours are 7:00 a.m. – 3:30 p.m. Monday – Friday (excluding
holidays).
2. An application for service must be completed and approved,
and the fee to establish the account must be paid for the account
to be activated.
3. Payment must be made or a payment agreement signed for any
outstanding bills.
4. The completed application may be

application. (Official Payments charges a $3.95 fee for each

office or over the phone.
